Acute Major Peripheral Vascular Injuries in Vietnam War



Abstract
34 cases of major peripheral vascular injuries among 1,077 cases who were admitted to the department of General Surgery of ROK MASH, Vung Tau, Vietnam during these 3 years and 7 months from Oct. 1964 to May 1968 were analysed.
1. 46 vascular injuries were seen in the 34 casses, in which 12 cases had two vascular injuries of 46 vascular injuries 35 were arterial injuries and 11 were venous injuries.
2. Of 35 arterial injuries 10 were femoral artery which was the cammonest in frequency, and the next was 8 cases of brachial arteries of 11 venous injuries 5 were femoral vein, 2 were internal jugular vein and 2 were brachial vein.
3. Most of the vascular injuries were caused by war wounds such as gun shot and shell fragment wonds.
4. Vascular reconstruction was carrdied out by means of end-to-end anastomosis for 22, lateral repair for 7 saphenous vein graft for 3 and for 3 and Dacron prosthesis for one vasular injury. 13 vascular injuries were primarily ligated because the reconstructive surgery was inadequate.
5. The results of the the treatment were analysed in 3 groups such as "Good", "Fair" and "Failure". Good was 24, fair 5 and failure 5 respectively. of 34 cases, 6 were dead mostly due to associating injuries and hypovolemic shock.
6. Analysis of "Failure" cases were made. We foud the results were intimately related to (1) lag period from injury to surgery. (2) state of shock, (3) severe soft tissue injury and bone fracture, (4) location of the vascular injury, (5) state of back-bleeding from the distal end of the injured vessel and (6) postoperative wound infection.
